Professional fees

Comprehensive Eye Exam

  • Comprehensive Eye exam : $110
  • New contact lens patient $150
  • Existing contact lens  Patient: $125

Vision Therapy Services

  • Referral Office Visit: starts at $125
  • Developmental/Perceptual Work-Up: $250  (includes consultation and report)
  • 1 Hour Vision Therapy Session: $125

Other Services

  • Brief office visit $40
  • Moderate office visit $88
  • Extended office visit $126

Fees are expected to be received at the time of service or when materials are ordered. We accept all major credit cards, checks and cash. We also accept financing through Care Credit

Health Care Insurance

We are currently providers with many health plans. Please check with your employer or look on your insurance card to see if we are participating in your plan. If If you do not have one of these plans, we will be happy to bill your plan on your behalf

Please notify us of all vision and medical health care plans you are enrolled in so our office staff can assist you to obtain the maximum benefits specified by your insurance. The help we give does not relieve you of the following responsibilities:

  1. Your vision benefit program is a contract between you, your employer and the insurance company. We are not party to this contract. Our office files your insurance as courtesy to you.
  2. You (not the insurance company) are responsible to us for all fees for services rendered to you.
  3. Not all vision services are a covered benefit in all contracts. It is your responsibility as the patient to contact your insurance to determine eligibility and coverage
  4. The amount of the benefits that the insurance company is expected to pay is an estimate only. It is not a release of your responsibility for full payment of services rendered
  5. if we are not a provider with your health plan, you may be asked to pay the full amount at the time of service, depending on our experience with your plan
